How do I establish credit with Clark Environmental?

Under the forms tab, click on the credit application document under the generator forms section.  After downloading the form from our site fill out the information and fax the information to Accounts Receivable (863.744.2200).  Please give fax numbers for your references.

How do I get a copy of your insurance?

Under the forms tab, click on the insurance certificate document under the certificates section.

How do you find / fill out the profile document?

Under the forms tab, click on the hazardous waste generator profile document under the generator forms section. After downloading the form from our site fill out all information you know. Pages 3-4 from the profile document have detailed instructions.

Note: You cannot save data typed into this document. If you would like a copy for your records, please print out the form when completed.

How early & how late can we bring in trucks?

CLARK is open from 8 AM to 4:30 PM Monday thru Friday (weekends and holidays closed). If you have an emergency and need to come in earlier or later, please call our office. We will do what we can to accommodate our customers.
